Welcome to the Champaign County (Illinois) Mental Health Board (CCMHB) and Developmental Disabilities Board (CCDDB) Registration, Application, and Reporting System.

The basis for the CCDDB's mission and policies is the Community Care  for Persons with Developmental Disabilities Act, 50 Illinois Compiled Statutes 835.

The basis for the CCMHB's mission and policies is the Community Mental Health Act, 405 Illinois Compiled Statutes 20.

To promote supports and services for residents of Champaign County, Illinois, the Boards also fund: 211 information and referral; program evaluation support and related workshops; learning opportunities for providers; Cultural and Linguistic Competence and Mental) Health First Aid training; anti-stigma awareness and support for artists with lived experience or a disability; and a disABILITY Resource Expo.

The Executive Directors of agencies that have successfully completed the registration process will receive an e-mail containing login credentials.

Instructions

Registration Instructions for Agencies:

  1. Enter your agency's name. Indicate the intended funding source - Champaign County Mental Health Board, Champaign County Developmental Disabilities Board, or I/DD Special Initiatives. You will be issued a registration ID which can be used to return to complete the registration process if necessary.
  2. Review and acknowledge that you have read the Funding Requirements and Guidelines.
  3. Complete the online eligibility questionnaire(s) for the funding source(s) you have chosen. There are many questions, so feel free to print the form and later return to this online system to complete the form with your responses. Remember to press "Save my Responses" to avoid having to re-enter data.
  4. Submit the eligibility questionnaire(s) to the Board(s).
  5. Choose a username and submit your completed registration.
  6. Note: Before completing the last step, you may return as often as necessary by using the registration ID issued in Step 1.

Upon completion of the registration process, a password will be e-mailed so that your agency can log in and use the features of the system. Your Executive Director will be able to create additional users with appropriate access to this system.

Be sure to update the Agency Details section with the contact information of the Director and any other staff to be notified. Also in the Agency Details section is a Board Member List. Complete this and update as any of the required information changes.
